It's 2020 and I've never seen this before. :o

Does anybody know anything about this, for instance, how many were made?

I believe Bloodshot himself, Jesse Fresco, made that. Not 100% sure but I’ll ask around.

Yes, the cosplayer. I also believe Jesse Fresco set this up, I remember seeing something about this a while back on FB.

Tried to reproduce this using GIMP -- based on LaRosa's cover for Bloodshot Reborn #1.

Had to add a boot, remove the holster and some muscle below the armpit.

Decided to keep the upper text strictly black & white.

Maybe I'll do another version tomorrow with the rest of the text and the logo in black & white also.
